We Welcome New and Established Patients of all Ages to Our Practice

Your primary care provider may refer you to us or you may self refer for routine or problem care or for a second opinion.

You may also request an appointment or obtain additional information about our practice by emailing us at Be certain to include your full name, phone number(s) and all relevant information so that we may easily contact you. Please be aware that email is for scheduling or information purposes only. We will not dispense medical information via email as we feel this is an unsafe way to communicate about your health care.

Please bring a list of your medications, your completed paperwork (see Important Forms) and your insurance information with you. If you are unable to complete your paperwork before your visit, please arrive 15 minutes early so that you may do so.

Please be aware that a copay is collected before your visit. If you do not have insurance, we are pleased to offer a 20% discount for cash payment at the time of service. If this is not possible for you, we will ask you to meet with our practice manager to set up a payment plan that will work for you

If you are transferring your care to Vermont Urogynecology from another practice, please complete the Records Release Authorization Form so that we may obtain your records and review them prior to your visit.
